Volunteer Forms
If you are 18+ years old, you will need:
If you are below the age of 18, you will need:
- A filled-out Volunteer Application Package form, preferably in a physical copy.
- A Criminal Record Check - this can be obtained at Winnipeg Police Service (245 Smith Street, right behind the Millennium Library).
- A Child Abuse Registry Check - this form is already included in the Volunteer Application Package; submit it with the rest of the Package to us, and we can have it processed for FREE.
- 2 pieces of government-issued ID.
If you are below the age of 18, you will need:
- A filled-out Junior Volunteer Application form signed by you and your parent/guardian.
